I (parent/guardian) hereby release, indemnify and hold harmless the American Choral Directors Association of Minnesota (ACDA-MN), its trustees, employees, volunteer workers, students, agents and assigns from any and all liability, damage, claim of any nature whatsoever arising out of or in any way related to my/my child’s participating in the ACDA-MN Honor Choir program. Participating in any activity is an acceptance of some risk of injury. I agree that my/my child’s safety is primarily dependent upon him/her taking proper care of self. Despite precautions, accidents and injuries may occur and injury and/or loss or damage to personal property may occur as a result of participation in the ACDA-MN Honor Choir; therefore, I assume all risks related to participation in the ACDA-MN Honor Choir. I also hereby acknowledge that the American Choral Directors Association of Minnesota (ACDA-MN), its trustees, employees, volunteer workers, students, agents and assigns assume no liability whatsoever for personal injuries or property damage that may arise out of my/my child’s participation in the ACDA-MN Honor Choir program.
Permission to use Photo/Video/recording files(Required)
I (parent/guardian) hereby grant permission for the release of photographic images and/or video files of the State Honor Choir to be used for current and/or future promotional and marketing purposes (including social media), and/or program documentation, directly affiliated with the American Choral Directors Association of Minnesota.
Student Behavior Contract(Required)
As a selected member of the ACDA-MN State Honor Choir program, I (participating student) will be expected to follow these basic expectations of conduct and behavior that will bring honor to my school/choral organization, my teacher, my parents, and myself. • I will listen attentively to all adult members of the ACDA-MN honor choir staff and will follow their guidance and instructions. • At all times throughout the day, I will be courteous to all of my fellow student singers, treat them with dignity and respect, and will accept their unique differences without bullying, harassment, or negative personal judgment. • When my section is not singing, I will observe the conductor and listen to what the other singers are doing to make our performance the best it can be. • I will turn off all electronic devices and leave them out of reach during rehearsals and performance. • Drinking containers filled with water are the beverage allowed in the rehearsal area. I will consume food items only in the designated facilities during breaks and lunch time. • I will not wear hats. This shows respect for our beautiful facilities, our guest conductor and staff, as well as the privilege of being selected to this prestigious honor choir program. • As a guest, I will respect and personally help care for all of the rehearsal and performance facilities throughout the day. • I will be responsible for rehearsing and preparing the music to the best of my ability, and will strive to give 100% of my talent and ability to make my honor choir experience successful. By checking the box below, I acknowledge that I have read, understand, and will follow these expectations for my behavior during honor choir day. Further, I understand that consequences for not following these expectations will range from verbal warnings to being removed from the choir. My parents will be notified as discipline problems occur.
